Eligibility and Cost

Mountain View PACE serves seniors from all walks of life who meet the following eligibility requirements:

  • 55 years and older

  • Live in designated counties served

  • Meet state nursing facility level of care criteria

  • Live safely in the community with PACE services at the time of enrollment

Mountain View PACE does not discriminate in the delivery of PACE services based on race, ethnicity, national origin, religion, sex, sexual orientation, age, mental or physical disabilities, or source of payment. Register with us today.

How is PACE paid for

There is no charge to you or your family for any drug, service, or care approved by your Mountain View PACE health care team.*

*Costs for Mountain View PACE services may be your financial responsibility, depending upon your Medicare and Medicaid eligibility. If you do not have Medicare or Medicaid, services may be paid privately. Because enrollment in PACE is voluntary, enrollees may choose to voluntarily disenroll from the Mountain View PACE program and resume their care in regular Medicare and Medicaid programs.

Mountain View PACE accepts Medicaid, Medicare, and private payments.

  • Medicaid only, no premium

  • Medicare and Medicaid, no premium

  • Medicare but not Medicaid, monthly premium

  • Non-Medicare or Medicaid-qualified monthly premium

Mountain View PACE participants must receive all needed healthcare, including primary care and special physician services (other than emergency services) from Mountain View PACE contracted providers.

A day in the life


Mountain View PACE exists to help seniors navigate the complicated issues associated with aging. Our unique program is customized for each participant, with the goal of helping them continue living independently in their homes and active in their community.

What a day might look like at Mountain View Day Center

The Mountain View PACE Day Center is an active place where seniors have access to medical care, meals, therapy, exercise, and social activities.

Participants come to the Day Center based on their healthcare team’s recommendations and the needs and interests of each participant.

Your morning begins when the Mountain View PACE van arrives at your front door. You’ll come to know our friendly and professional van driver who will help you in and out of the van safely and comfortably.

If needed, our driver will help you bring your laundry to the Mountain View PACE Day Center while you enjoy your day outing.

When you arrive at the Mountain View PACE Day Center, you may visit with the medical staff, socialize with friends, enjoy a lunch specifically for your dietary needs, or exercise with a physical therapist to maintain your strength and mobility.

Additional events and activities at the Day Center may include entertainment, board games, or a quiet talk with a social worker. After a full day at the Mountain View PACE Day Center, our van driver will drive you to your home.
