Disenrollment Information

The Mountain View PACE program is voluntary for the individuals that choose to enroll in Mountain View PACE. Because of this, a participant may voluntarily dis-enroll from Mountain View PACE by communicating this to the Mountain View PACE team in writing with as much advance notice as possible. Even if shorter notice is provided, all efforts will be made to dis-enroll as soon as possible.

Until a participant is dis-enrolled from Mountain View PACE, all services provided for the participant must continue to be authorized by Mountain View PACE as Mountain View PACE remains liable for payment. The Mountain View PACE team will coordinate all care planning activities to primary care and other community resources that the participant may be discharging to.
